Please read my faceup policies first!

Here you will find the generic form that will need to be filled out when you approved for a commission slot. You are free to give extra information or choose not to answer some of the questions that are not required if you are unsure, or if you would rather describe the character to painted rather than the specifics of the faceup. After being confirmed on the waitlist, you can email a copy of this form, filled out to your discretion. There are also a word document and PDF version of this form at your disposal.
